- Mount Olivet Home, a 94-bed facility in Minneapolis.
- The Villa at Bryn Mawr, a 112 bed for-profit facility in Minneapolis, which also received a “Much Below Average” overall rating.
- The Villa at Osseo, a 115-bed for-profit facility in Osseo, Minnesota, which also received a “Much Below Average” overall rating.
Medicare has rated the staffing at the following six Minnesota nursing homes as “Below Average” (the second worst rating):
- Andrew Residence, a 212-bed for profit facility in Minneapolis.
- Bywood East Health Care, a 101-bed for-profit facility in Minneapolis.
- Camden Care Center, a 87-bed for-profit facility in Minneapolis.
- Elliot Care Home, Inc., a 15-bed for profit facility in Minneapolis.
