MDH Finds Apple Valley Assisted Living Facility to Have Violated State Licensing Laws

The Minnesota Department of Health (MDH) has completed its investigation of neglect concerns at the Centennial House of Apple Valley in Apple Valley, Minnesota. A complaint was filed with the Department alleging that a resident of the facility called 911 when she could not find a staff member to assist. The department investigated the matter and found that once police arrived to the facility after the 911 call, they found a staff member on duty who was asleep. The employee admitted to being under the influence of alcohol and pain medication. The MDH determined that the facility failed to provide care to its residents by certified caretakers and is responsible for the neglect of the facility’s residents. (Case no.  HL24139009)
