The Minnesota Department of Health (MDH) has completed its investigation of neglect concerns at Cerenity Care Center on Humboldt, a nursing home in St. Paul, Minnesota. A complaint was filed alleging that neglect occurred when a resident sustained a leg fracture when the resident was transferred by an employee of the facility in a manner not consistent with the resident’s care plan. The Department investigated the matter and found that there was enough evidence to substantiate the claim. The resident had a history of multiple fractures and the resident’s care plan required that three staff members use a mechanical lift for all transfers. Facility documentation revealed that the resident had a bruise of unknown origin. Document review confirmed that only two staff members were present when transferring the resident to the mechanical lift. As a result, the resident’s foot got caught in the sling, resulting in the fracture. [Case no. H5255035]
