The Minnesota Department of Health (MDH) has completed its investigation of abuse concerns at St. Raphael’s Health and Rehab in Eveleth, MN. A complaint was filed with the Department alleging that an employee of the facility slapped a resident in the face and leg. The Department investigated the matter and interviewed an employee who was at the location of incidence. The employee verified that it sound as if the resident had been slapped. Other employees interviewed reported that the offending employee told them that the employee had slapped the resident across the face. The MDH investigation substantiated the occurrence of the abuse.
