Details about the Jourdain/Perpich Extended Care Center Nursing Home in Minnesota

This 47-bed nursing home received “much below average” ratings for its Overall, Health Inspection, and Quality Measure ratings, according to Medicare’s 2012 annual report. Compared to the average number of health deficiencies for Minnesota nursing homes (6.6), this facility was cited with 19 health deficiencies. These include the facility’s:

  • Failure to hire only people with no legal history of abusing, neglecting, or mistreating residents, or report and investigate any acts or reports of abuse, neglect, or mistreatment of residents;
  • Failure to develop and implement policies for screening and training employees, and the prevention, identification, investigation, and reporting of any abuse, neglect mistreatment, and misappropriation of property;
  • Failure to provide care by qualified persons according to each resident’s written plan of care;
  • Failure to properly care for residents needing special services, including: injections, colostomy, ureostomy, ileostomy, tracheostomy care, tracheal suctioning, respiratory care, foot care, and prostheses; and
  • Failure to have a program that investigates, controls and keeps infection from spreading.

Medicare has also identified 11 instances of complaints and incidents associated with Jourdain/Perpich Extended Care Center. These include:

  • Failure to properly hold, secure, and manage each resident’s personal money which is deposited with the nursing home;
  • Failure to provide timely notification to the resident before transfer or discharge;
  • Failure to tell the resident or the resident’s representative in writing how long the nursing home will hold the resident’s bed in cases of transfer to a hospital or therapeutic leave;
  • Failure to use a registered nurse at least 8 hours a day, 7 days a week; and
  • Failure to make sure that the facility is administered in an acceptable way that maintains the wellbeing of each resident.

Further, the Minnesota Department of Health has substantiated 2 reports of nursing home malpractice on the part of the facility, including its

  • In December 2011, MDH substantiated a report of a violation of a resident’s rights when a resident was not provided with a proper notice of discharge. [Report no. H5535003]
  • In December 2012, MDH substantiated a report of under-staffing employees of the facility. As a result, multiple residents have had falls with injuries, and residents have expressed concerns about their care and safety. Additionally due to understaffing: residents of the facility were not able to obtain personal funds on evenings and weekends; the temperature of the facility was not maintained to comfortable levels in the rooms of residences; and the facility failed to provide a licensed nursing home administrator to address system failures regarding resident rights and grievances, staffing, maintenance, and vulnerable adult reporting.
