Neglect

REM Repeatedly Cited by State for Deficient Care

The Minnesota Department of Health has determined that the REM River Bluffs Rochester NW in Rochester is responsible for neglect of a resident.  A complaint was filed with the Department alleging that the resident was walking down the block near the facility without supervision. When he was returned to the facility, the gate to the fenced yard was standing open. Staff indicated door alarms had not sounded when he exited the facility. The Department investigated the matter and found that neglect occurred when the facility failed to supervise the resident.  [Report HG320002, dated May 15, 2012]

The Minnesota Department of Health also determined that the REM Hennepin Inc. – Lyndale in Minneapolis is responsible for neglect of a resident.  A complaint was filed with the Department alleging that the resident was not adequately supervised and was allowed to go outside with a visitor. The resident engaged in sexual activity with the visitor. The Department investigated the matter and found that neglect occurred when the resident went outside of the facility unsupervised and engaged in sexual activity with a visitor while on the facility grounds.  [Report HG202004, dated April 30, 2012]

The Minnesota Department of Health also determined that the REM Southwest Services Marshall B in Marshall is responsible for neglect of a resident.  A complaint was filed with the Department alleging that the perpetrator threatened and hollered at a resident. The resident expressed fear that the perpetrator would harm him and requested to leave the facility. The Department investigated the matter and found that the resident was emotionally abused by the perpetrator when she raised her voice and threatened him.  [Report HG146003, dated April 24, 2012]
