The Lexington Herald-Leader reports that state investigators have cited a Falmouth care facility that failed to check on one of its brain-injuried residents for nearly three hours. The facility had no policy to ensure that its staff provided continuous supervision. The resident’s body was found along a river bank about a month after he went missing from the facility.
